The following forms are required by Common Ground for all travelers working with our agency
Travel Forms
To start the arrangements for Cuba, all passengers should read the Travel Restrictions, and then complete the appropirate applicatin form and return it with original copy of our Travel Affidavit and a clear copy of your passport (valid 6 months beyond date of return from trip) to Common Ground. If traveling via Miami, you will also need to complete the flight reservation form for the specific flight charter company with which you are flying (see links to forms, below) and return an original copy to Common Ground. The visa form is included in the application form specific to your trip. To expedite, you may fax the forms before mailing them to us. See contact information.
Individuals
Please fill out the Individual Application form to begin making travel arrangements to Cuba and return to Common Ground with a Travel Affidavit .
Research Program
Please review the criteria for this new program and complete an Research Project Application as well as the Travel Affidavit .
.Groups
Please contact us if you are planning to take a group to Cuba. We will assist you in planning the trip and understanding and following the U.S. government travel requirements. Members of the group will need to complete an application form, visa form and travel affidavit, which we will provide once the trip is set up.

Obtaining or Revalidating Your Cuban Passport
NOTE: To reach the Cuban Consulate, click here. Common Ground is a private agency and does not have access to passport applications or other services you have requested from the Consulate. We can tell you that new passports generally take 6-8 months; renewals 2-3 months.
